It looks like my the rear of my frame might be a little out of whack, '92 short, the front of the bed sits level with the cab lines but the rear bumper sits about 1 inch higher on the right and its noticeable when looking from the rear to.
I can tell there used to be a frame mount hitch on it and it was on a farm for a while so maybe it was worked a little too hard one day...

Check the bumper brackets and the bed supports. Without a pic I'd guess that a bed support is collapsed and likely not the frame as the bed supports are significantly thinner than the frame rails.

Do you get a level bubble when you check the tailgate section as well?

If so, I would give the bumper & brackets a hard first look.

All looks good under the bed, no collapsed supports and I get the same reading on the level with the top of tailgate and top of bumper.

Gonna need to take measurements on both sides and mark when they start to really change. the bed needs to come off when I put in new cab corners so I'll wait till then to get a better look and try to figure out how to fix it.

I think this might be on my mind a little too much when I am lying in bed thinking of how to build and anchor spot in my garage floor to straighten it....
Or I could build on the driveway because it needs replaced anyway.

Seeing as how the bumper brackets bolt to the frame, I don't think you have a frame issue. If the frame was bent, the bed and the bumper would both be out of whack. Since it's just the bumper, I'd look at the brackets, one is probably bent a little.

No, the bumper and the bed are parallel with each other, I noticed the this when I measured the wheel arch before lowering it and the left rear sits almost one inch higher than the right and it's noticeable looking at it from behind.

Then I would look at the suspension as well. Those leaf springs may have taken a beating and will wear out before the frame twists. You probably have a broken or worn out spring pack.

Well, a spring would be easier than frame work.
