Well ..... if I had to guess I would say the hex was bottomed and maybe the seal wasnt fully seated (depth). Hard to be sure.

In the end i threw on a 2nd oring and used the thread sealant here (says hydraulic)

No more leaks ...... and can turn the wheels on the ground easily with single hand.

Will monitor and update.
