Not all spas require a neutral wire. Local codes will dictate if you need a GFI regardless, even though it will only work as a regular breaker. I'm not an electrician... but I stayed in a Holiday Inn Express last night!

If that 20 is feeding the hot tub, it needs to have a GFCI breaker or a GFCI at the other end (as where the hot tub plugs in). Should a short occur and a person has become the ground or the neutral, a standard breaker will see you as a "load" and will not trip until enough current is passing through the body for there to be an overload which will usually be higher than 20 amps.
You really want to verify that GFI in that circuit. And yes... Holiday Inn Express has a great hot tub and pool!

Thanks guys.
I believe he rewired so that the new 220V spa runs off the GFCI double pole . I think the old 110V spa ran off that 20A circuit. Now its just the exterior landscape lights.
