It is a chore to get it out for sure. It's glued in. I used a strap wrench and a chisel to get it started. In your case it might be a good idea to insert something inside the outlet tube first.
AD, it's not really an issue of fender width, keep in mind, my engine is under the rear part of the cab. My only limiting issue is frame width and that is wide enough to fit the Silverado radiator between the rails. Above the rails there was no limit just a matter of how far I wanted to open up the back of the cab. The other plus is exhaust clearance is no issue.
