License plate frame hardware

Can anyone tell me how the license plate bracket on the rear bumper is supposed to held on? Someone messed with mine before I bought it and I have only the outer two carriage bolts. What goes in the top? Pictures help!

The box is held in by 4 carriage bolts. Two on the top and one on either side. They look just like the ones for the bumper.

I only have two holding mine in right now. I have been too lazy to buy the hardware.
