Tilt Column Shifts Out of Park Without Using Column Shift Lever

I noticed months ago - while I was working in the engine bay - that this tilt column will shift out of park without pulling the shift lever on the column. Pretty scary, I thought and I will have to be wary of this while working on/under the truck.

Where to I need to look for the problem? I've got the column torn down to replace the turn signal switch and am going to just rebuild it while I doing it. New bearings are in order and I was surprised to have to remove a mud-dobber's nest out of the hub.

Thanks, I'm guessing it is as simple as that! The pointed end on my shift lever isn't square, but appears to be worn at an angle.

I just rebuilt my steering column and while I had everything apart I gave the shifter selector a couple tack welds and spent 10 mins filing it down. It certainly had some wear on it.
