As already mentioned, that is an extendable bumper for a camper that appears to have been reinforced for more capacity.

Here are a couple pictures which show the one I had the dealer install when we purchased the truck in 1969. I have modified it several times for use on a 1986 Chevrolet dually and 1999 Ford F-350 as well as to add a hitch and capacity. It was last used with the original camper in 2001.
