Yesterday took a rear wheel of my 68 Golden to find the lug shoulders warn badly . Also the rim holes were warn ,my guess is the lugs bottomed out on the drum after repeated changes and over torquing over the last 53 years causing the rim to be loose enough to move and wipeout the holes. Not using the originals anyway .

Another possible cause...It does not long for a loose wheel to waller out the wheel holes and bugger the nuts.
