Unhappy Rusty Frame and Tracking Bar Mount

Hey guys! Back once again with another question. I have a few potential problems at hand. I recently noticed the rear tracking arm mount (I think that's what it's called) is about 50% rusted through on my 68 C20, along with this thumb size hole past the rear end in the frame.

My questions!

Have any of you had similar problems?

If so, how did you address them? Not quite sure how to fix this.

Anything can be fixed.
